The CCD Foundation Consent For Release of Student Financial Aid Information Form authorizes the Â鶹Éç of Denver and the Â鶹Éç of Denver Foundation to share students’ Federal Financial Aid, State Financial Aid, and Education record information with the scholarship donor(s). This form applies to scholarships administered by the Â鶹Éç of Denver Foundation and must be submitted directly to the Financial Aid Office.
